- We have a easy to use online booking platform which will link you to the medical form to complete prior to attending the appointment, please ensure you complete all the sections before attending the appointment.
- Please ensure you declare any medical conditions, tests or procedures during the assessment. Failure to do so may result if delaying your assessment process or denial of the certificate.
- If you have a result or finding which means you are not deemed FIT on the day, then the Dr will explain what information or treatment will be required before you can be made fit. In these circumstances, you will not receive a certificate on the day, this will be either deferred or temporarily unfit.
- Please understand that you will not be entitled to a refund if you fail your medical assessment.
- Photo ID will be required, e.g. passport, photocard driving license.
- Your local doctor’s name, address and contact details
- Please bring with your you up to date medical records from your NHS GP (this can be in digital form), and any letters and reports from specialists.
- You do not need to fast on the day unless you have been asked to have a blood test to assess your cholesterol or diabetes risk, in which case please fast for 10-12 hours prior to the blood test (NO food, tea or coffee but please DO drink lots of water and stay hydrated)
- If you need to complete a Chester Step test then please wear comfortable clothing and shoes.
- Please bring your glasses or contact lenses but not colour correction lenses.
If further tests are required, please contact the clinic in advance. We provide blood tests, urine drug screening, chest x-ray referral service, resting ECGs (heart tracing) at additional costs.
