If you have been requested to complete a medical form for either work or personal circumstances, you should always approach your NHS GP practice in the first instance as they have access to your medical records. However, if this is not possible then Dr Naseem (Private GP in London) is experienced in conducting various private medicals and able to help full requirements.

• Medical history review (please attend with NHS GP records/access to NHS app)
• Full systems examination
• Completion of form with stamp

Note: Additional tests come with additional charge and will lengthen the appointment time. If you require multiple tests, please contact  us in advance to ensure we block enough time in the diary for your tests to be completed

• Blood tests 
• Urine test
• Urine drug screen
• Alcohol breath test
• Resting 12 lead ECG
• Audiogram
• Vision tests (Snellen chart acuity  and Ishihara's colour vision test)
• Spirometry (lung function test)
